BEHAVIORAL INTERVENTIONIST (PROJECT FABULOUS)

Behavioral Interventionist responsible for facilitating transgender population interventions by identifying HIV/AIDS at-risk individuals, providing education & counseling, developing individual prevention care plans, ensuring interventions & topics are covered based on client needs & readiness; & administering base-line surveys during intake process for measurement of client's knowledge & risk behaviors. Bachelor's degree in Social Work, Psychology, or  a related field. Experience in providing case management, risk reduction, behavioral interventions & crisis counseling services to HIV positive populations. Knowledge of HIV/AIDS, particularly related to the transgender community preferred. Starting annual salary $30k, w/benefits.

PSYCHO-SOCIAL COUNSELOR  (DROP IN CENTER)

Responsible for providing psycho-social evaluations & counseling to clients in HERO's day & respite care center; conducts intake to ensure eligibility for services & crisis intervention services; provides substance abuse intervention making referrals to appropriate agencies; assists with individual & group substance abuse counseling; & reviews quality & appropriateness of client activities. Candidate must possess Bachelor's degree in Social Work, psychology or a related field, with at least three years of social work or psychosocial counseling experience, along with crisis counseling, substance abuse counseling procedures & knowledge of community resources & referral agencies. CSC-AD, a plus. Starting annual salary $30k-$35k w/benefits. .

CASE MANAGER - LSWA

Responsible for providing case management services to a maximum caseload of 65 or more clients; maintains documentation, updated care plans, psychosocial assessments & on-going progress notes on each client; Makes home & hospital visits as necessary; provides triage, walk-in intake, crisis intervention & emergency community meetings, as required; reports monthly data & statistics of case load depending on reporting program/grant requirements; Candidate must possess a BSW and LSWA, with experience providing case management, linkage referrals & crisis counseling. Knowledge of drug/alcohol addictions, a plus. Starting annual salary $26k to$29k w/benefits.

PROJECT COORDINATOR/AT-THE-DOOR CASE MANAGER - LGSW

Responsible for management of all daily activities & operations of the Hometies case management program & staff, and provides At-the-Door case management to discharged HIV clients from prison populations. Communicates & meets with community linkages & other services providers; reports monthly data & statistics based on reporting requirements; case manages a maximum caseload of 75 or more clients; connects clients to needed long- & short-term support services; monitors client's medical condition including interaction with medical practitioners; coordinates discharge planning with prison social workers three months prior to release; completes comprehensive psychosocial assessments & goals/activities plans with client; identifies appropriate transitional housing programs; assists clients in housing, substance abuse, medical, financial, psychiatric, social, transportation, legal, education, employment or any other psychosocial issue/need. Candidate must possess MSW and LGSW (minimum), with at least three years of social work or psychosocial counseling experience. CSC-AD, a plus. Knowledge of crisis counseling, substance abuse counseling, & community resources/referrals. Starting annual salary $35k-$38k w/benefits.

CASE MANAGEMENT/ADHERENCE SPECIALIST (CASE MANAGER - LCSW)

Responsible for supervision of Ryan White (RW) program staff, overseeing management of daily activities/operations & acts as liaison with HERO's Primary Care Clinic staff/clients. Also provides direct case management services to clients & implements SOAR Program; develops & evaluates quality assurance, performance improvements & quality management activities of RW Title I & II programs; responsible for triage, SOAR applications & crisis interventions; & carries a caseload of 30 medially complicated fragile clients from HERO's Primary Care Clinic. Candidate must possess MSW and LCSW, with experience in managing case management services. Knowledge of case management principles, supervision /administrative principles, financial assistance & community HIV resources. Starting annual salary $40k - $45k w/benefits.
